Lets take Leon for example. His jacket model is Pl00. In the em folder there is a Pl00 folder and within it are several .pmd's. These are the model files. You'll see files ending with a to l (e.g., pl000a.pmd, pl000b.pmd).
Breakdown of each:
Pl000A - Main Body (arms chest and animation Skeleton)
Pl000B - Head eyes mouth no face
Pl000C - face
Pl000D - Hair
Pl000E - Chest (jacket)
Pl000F - No F in Leons folder for some reason.
Pl000G - Neck
Pl000H - Head and hair *???* (Decapitation?)
Pl000I - Head and hair *Melted Face*
Pl000J - Knife
Pl000K - Small ploy (cheek cut)
Pl000L - Holstered knife Handle (Part as other half is on jacket)
The RH and LH are leon's hands.
You can make any one of these body parts dissapear without any problem by creating a blank text file renamed .pmd with the files name.
Leon's other costumes tend to pull the head/neck/face and hand files from this folder so changing leon's head will result in a change for all.
Now most of the playable characters follow this format to some degree. Maybe not having certain things like a knife model or a melted face model.
NPC
Now look at Ashley in pl01.
Pl0100 - Full Ashley (complete bar hair)
Pl0101 - Ashley's Hair
She can be swapped to leon with a few more complications. More later.
NPC layout may vary as the policia have a body/cap model and a head model rather than a full body+head and hair like Ashley. This makes it next to impossible to switch a model into Ashley's slot because they are made up of broken down models (though some do have a full head hair model but it is at the wrong height).

If you want to find out what a model is, I suggest using Ashley as a guinny Pig. Have a save fille with ashley ready. Copy the textures from the folder into Ashley's folder, replace either Ashley's hair or body with a blank textfile rename to the pmds name and the model part you with to find out rename to the other ashley file and then run the game and it shall show you what is contained within the file.

Switching Models Around
it is simple to switch the playable characters around. Just take the textures from the playable character and place them in the folder of the model you wish to change then replace the pmds in that folder from the donar model. Any body part you don't need. Just use the blank pmd on it.
As for NPCs, like the policia. copy the body over the A.pmd and the head over the C.pmd as there isn't any hair. Replace the rest of the unneccessary files with blank pmds.
As for Ashley, there are two methods:
1. Replace the A.pmd with Ashleys 0.pmd and replace the d.pmd with ashleys 1.pmd and blank the rest. With this method her moving parts will appear in the room static and you'll get horrid stretched polygons. give it a go and see what I mead.
2. The better version. Renaming Ashleys body to e.pmd (usually an exrea part of the model but you cn't just blank the A.pmd as the model will just be invisible (as I believe the A.pmd sets the models skeleton). Instead use ashley's hair as A.pmd and put ashleys body as e.pmd (I believe you can use other parts of leons models as donar slots) and also put ashley's hair as D.pmd (as using it as a skeletal donar will make the hair dissapear). This will irradicate the stretched polygons but will make ashley's hair act similar to Leon's.
Remember: Any bodypart can be swapped into another location (i.e. Leon's Neck or leon's eyes pmds) as each model has a designated height to start at) but there must be a body model in the A.pmd slot, else the animations go to pot.
