That's right. Tomas has figured out how to make the game spawn a different item in place of another.

When you extract one of the ST dats, you're given the room dats. Extract them too, using Vampyre's dat extractor. Then you open your hex editor and change a value within the ITA file.

I'll be using the laser room as an example.

Somewhere in the file will be the number A1. This is the emerald that can be found by Saddler's chair.

You can change this number to change whatever appears. Here's a list of item numbers currently known. I'll update this whenever I can. Item positioning hasn't been figured out yet.
